Ideal Candidate

• Holds a master's degree in social work or counseling psychology
• Has at least 1 year of experience as a Social Worker or Counselor
• Preferably has 2 years of hospital experience

Job Description

Under the direction of the Case Management Director and with minimal supervision, the Inpatient Social Worker/Patient Counselor is responsible for:

• Assisting patients with bio-psychosocial needs
• Facilitating access to medical care and community resources
• Supporting patients in achieving their highest level of health

Key Responsibilities:

• Provide crisis intervention and short-term counseling for patients and families
• Coordinate discharge planning in collaboration with the care team, patient, and family
• Support patient transitions along the continuum of care
• Connect patients to both internal services and external community resources

Required Qualifications

• Master’s degree in social work or counseling psychology
• Minimum of 1 year of experience in a social work or counseling role

Preferred Qualification

• At least 2 years of experience in a hospital or inpatient setting
